Nurse Practitioner
NP’s are Regulated Health Professionals with additional university education in assessment and advanced decision-making skills to provide a full range of personalized , comprehensive and quality primary health care services to the rostered patients of the Great Northern Family Health Team. Nurse Practitioners can assess, diagnose and treat common illnesses and injuries and chronic diseases such as Diabetes and High Blood Pressure, order and interpret blood and diagnostic tests, write prescriptions and make referrals to other health professionals.

The Family Health Team Nurse Practitioner works in partnership with you, your family doctor, other members of the FHT health professional staff and anyone else involved in your care using comprehensive and coordinated team approach.

Registered Social Worker
Social Work services are available to patients of the FHT who are 16 years of age and older. Some issues that social workers deal with are depression, anxiety, grief, stress management and chronic pain.
